Refer an Athlete or Patient to True Performance

For physicians, athletic trainers and coaches. Send us a patient or athlete who needs rehabilitation for balance, vision, coordination or movement, and we keep you informed.

The short answer

True Performance is a chiropractic- and rehabilitation-led clinic in Rancho Santa Fe, CA. Care is led by Dr. Jeff McWhorter, D.C., a chiropractor whose practice focuses on functional neurology, concussion rehabilitation, and performance. We add measured rehabilitation alongside the care you already provide, and we never replace it.

Who refers

Who refers patients to us?

Physicians and other clinicians

For patients with lingering concussion symptoms, dizziness and balance problems, or people working to regain balance, coordination, and movement after a stroke or head injury.

Athletic trainers

For athletes who need structured rehabilitation after a head injury or a sports injury, or a pre-season baseline test.

Coaches and sports programs

For teams that want baseline concussion testing, and for athletes who need structured, step-by-step return-to-play programs, planned with their families.

What happens next

What happens after you refer?

  1. We contact the patient

We call the patient, or the parent of a minor, to set up a complimentary 15-minute intro call and the first evaluation.

  2. We request records

With the patient's written permission, we ask for your notes, imaging reports and any activity restrictions.

  3. We evaluate function

The first visit takes about 90 minutes, plus data and history gathering. It covers balance, eye movement, coordination, reaction time and, where useful, qEEG brain mapping.

  4. We report back

With permission, we share findings and the rehabilitation plan with you, and we update you as the plan changes.

Your role

What stays with you?

Diagnosis, medication, imaging and medical decisions stay with you and the patient's medical team. If our exam turns up something that needs medical attention, we send the patient back to you.

Questions

Common questions

Ask us: (858) 285-0073

Does the patient need a referral to be seen?

No referral is needed to book an evaluation. A referral helps us start from your findings and keeps you informed.

Do you see young athletes?

Yes, we work with kids of all ages. Parents take part in the full evaluation process and give written consent, and we plan the return to school and sport with families, coaches and athletic trainers.

Will you send me the findings?

Yes, with the patient's permission. We share the exam findings and the plan, and we update you when the plan changes.

Can you see patients who live out of town?

Yes. Many plans are scheduled around a visit, and our guide to visiting the clinic from out of town explains how the trip works.
